Robert E. Averbach, DDS, FICD, FACD, is a Professor of Endodontics, President’s Teaching Scholar, Outstanding Didactic Professor of the Year, and recipient of the prestigious Chancellor’s Award for Teaching Excellence at the University of Colorado School of Dentistry where he served as Dean from 1989 to 1998. A Diplomate of the American Board of Endodontics and a practicing endodontist, he is a prolific contributor to scientific journals and textbooks, a member of  the Scientific Advisory Board of the Journal of Endodontics, a reviewer for the Journal of the American Dental Association and a clinical consultant for THE DENTAL ADVISOR.

Brian Brada, DDS, MS, is Assistant Professor in the Departments of Restorative and Surgical Dentisty at Colorado School of Dentistry. His postgraduate experience includes Prosthodontic and Maxillofacial Prosthodontics training at the Wadsworth VA in Louisiana, residency at UCLA, 8 years teaching at Colorado University, 8 years as a general practitioner and a decade as a practicing specialist. Fixed and removable implant prosthodontics are his primary field of expertise. Dr. Brada has lectured extensively and presented numberous CE courses nationall and internationally.
Jon Fundingsland, scientific affairs manager, 3M ESPE, trained as a Chemical Engineer and joined 3M in 1979. During his years in research and development, Jon worked on such diverse product areas as the use of monoclonal antibodies to detect periodontal pathogens, dental implants, and the use of hydroxyapatite to repair osseous defects associated with periodontal disease. Most of his experience, however, was in the area of dental adhesives, resin modified glass ionomers, and composites. Jon has been granted 9 patents pertaining to various aspects of adhesives, and was on the development teams for such products as Scotchbond Multi-Purpose Plus, Vitrebond Liner Base, and Opal Luting Cement, as well as contributing to Single Bond and Filtek Supreme.
J. Mel Hawkins, DDS, BScD AN, FADSA, DADBA, a graduate of the University of Toronto Faculty of Dentistry with formal training in dental anesthesiology, is Professor of Pharmacology, Department Chair, Aurora Dental College, Aurora ON, Canada, Faculty of Medicine and Dentistry and the Founding Director of the Intravenous Sedation Program at the University of Toronto, Faculty of Dentistry. He has over 30 years private practice experience. Dr. Hawkins is Board certified as a Diplomate of the American Dental Board of Anesthesiologists and is a Fellow of the American Dental Society of Anesthesiology. He also holds fellowships with the International College of Dentists and the Pierre Fauchard International Academy and serves on the Continuing Education Committee for the American Dental Society of Anesthesiology. Jacob K. Lee, DDS, MS, was a faculty at Loma Linda University, Director of Graduate Program in Pediatric Dentistry at University of Minnesota, Chairperson of Pediatric Dentistry at the University of the Pacific School of Dentistry, in San Francisco, the Dental Director of the Children's Dental Health Clinic, and the Director of the USC Pediatric Dental Residency Program at Miller Children’s Hospital, Long Beach Memorial Medical Center. He is a Diplomate of the American Board of Pediatric Dentistry, a Fellow of the Royal College of Dentists of Canada, and is a member of the American Academy of Pediatric Dentistry's Council on Continuing Education. He is a consultant to numerous dental companies, and his publications and research interests are in improving dental materials for children. Dr. Lee is a continuing education presenter for national and international dental associations and meetings, and is the principle speaker for Leeway Pediatric Dentistry, a continuing dental education provider. Dr. Lee is engaged in private practice in San Clemente, CA.
John D. McDowell, DDS, MS, presently serves as the Director of Oral Medicine and Forensic Sciences at the University of Colorado School of Dental Medicine. He also holds academic appointments in the departments of medicine at the University of Colorado School of Medicine and the University of Utah School of Medicine. Dr. McDowell is also a fellow of the American College of Dentists and the International College of Dentists. Dr. McDowell has received multiple awards recognizing his teaching skills.
Kevin L. Rieck, DDS., MD, is a Consultant in the Department of Surgery, Division of Oral and Maxillofacial Surgery at Mayo Clinic. He is a Diplomate of the American Board of Oral and Maxillofacial Surgery and a Fellow of the American Academy of Cosmetic Surgery, AAOMS and ACOMS. His surgical interests include orthognathic surgery, cosmetic facial surgery, dental & craniofacial implants and associated hard tissue grafting procedures. Dr. Rieck enjoys many outdoor activities with his family including fishing, biking, scuba diving and other water sports. Together they all share an interest in Martial Arts. He is currently a Third Degree Black Belt in Tae Kwon Do.

Jamison Spencer, DMD, MS, is adjunct faculty at the Craniofacial Pain Center at Tufts University School of Dental Medicine. He is also adjunct faculty in the anatomy department at Boise State University. Dr. Spencer is a Fellow of the American Academy of Craniofacial Pain, a Diplomate of the American Board of Craniofacial Pain and currently serves as the President-Elect of the American Academy of Craniofacial Pain (becoming President in August of 2010). Dr. Spencer is a Diplomate of the Board of Dental Sleep Medicine and a credentialed member of the Academy of Dental Sleep Medicine. Dr. Spencer teaches head and neck anatomy at Boise State University and teaches courses to dentists and physicians nationally and internationally on the topics of craniofacial pain, TMJ disorders, head and neck anatomy and dental sleep medicine. Dr. Spencer is the director of the Craniofacial Pain Center of Idaho in Boise and the Craniofacial Pain Center of Colorado in Arvada. This course has been registered for 18 units of continuing education credits for each week. We are approved as a continuing education sponsor by the examining boards in California and other states. Dental Seminars & Symposia, LLC is designated as an Approved PACE Program Provider by the Academy of General Dentistry. The formal continuing education programs of this program provider are accepted by AGD for Fellowship, Mastership and Membership maintenance credit. Approval does not imply acceptance by a state or provincial board of dentistry or AGD endorsement. The current term of approval extends from June 1, 2010 to May 31, 2014.

Registration fee for each attending doctor is $1,030 for the first week and $1,795 to attend both weeks (see Special Savings For Hotel Guests). Any spouse or staff member who wishes to attend the seminar with their accompanying dentist will be charged $620 per person for the first week and $1,155 to attend both weeks. The first staff member from an office attending without an accompanying dentist must pay the doctor fee. Multiple dentists from the same office must each pay the doctor fee. Registration fee includes notebooks, morning refreshment breaks, certificates of attendance and other materials that may be provided. A $25 non-refundable deposit should be sent with your registration for each individual attending in order to reserve a space at the forum. All fees quoted are in U.S. dollars.

Refund & Final Payment
The $25.00 deposit for each individual attending the seminar is not refundable. There are no additional seminar cancellation penalties. Final  payment should be made by January 3, 2011. Make your tuition check payable to Dental Seminars and Symposia.

You may charge your tuition to your Visa, MasterCard, or American Express. No additional charge is made for this service. The refund policy is the same, except your refund will be credited to your charge card account.

We recommend purchasing your airline tickets well in advance in order to secure the best price. The cost for hotel accommodations begins at $xxxx per person, based on double occupancy for a seven night package. This includes hotel accommodations and related taxes. Luxury condominiums are available for those who do not want to stay at the full-service hotels. A minimum $500.00 per room deposit is required with your reservation; the balance is due 50 days prior to departure. An administrative fee of $100.00 will be assessed for any cancellation. Cancellations received 90 days or less prior to departure are subject to any additional penalties imposed by the suppliers. Trip insurance is recommended. Due to Hawaii's popularity as a meeting place, its facilities are at a premium during the favored winter months. The costs of the seminar meeting facilities are directly related to the number of participants staying at the seminar meeting hotel(s). As a result, each person attending the seminar can save $300 per week on their seminar fee and receive special low hotel group rates by staying at these hotels, with travel reservations made through Travel Leaders.
