Outstanding Faculty
Robert E. Averbach, DDS, FICD, FACD, is a Professor of Endodontics, President’s Teaching Scholar, Outstanding Didactic Professor of the Year, and recipient of the prestigious Chancellor’s Award for Teaching Excellence at the University of Colorado School of Dentistry where he served as Dean from 1989 to 1998. A Diplomate of the American Board of Endodontics and a practicing endodontist, he is a prolific contributor to scientific journals and textbooks, a member of the editorial board of Compendium, a reviewer for the Journal of the American Dental Association and a clinical consultant for THE DENTAL ADVISOR.
Vicky L. Crane, MA, with degrees in psychology and family science, has been adjunct faculty in the Sociology Dept. of UW-Eau Claire since 1983. She is past president of the Wisconsin Council on Family Relations. In 1996 Vicky and David Crane received the Lifetime Excellence Award from the UWEC Alumni Association for their quarter century of humanitarian work. She has presented programs internationally and is a nationally certified Quality Matters peer reviewer for quality assurance and excellence of online education.
Barry Goldenberg, DDS, MS, has been restoring
implants since 1986 in his St. Louis County private practice, which is limited
to Prosthodontics and Implant Reconstruction. During his lectures on restoring
dental implants, he explores computer utilization in the diagnosis and treatment
planning for implant dentistry. Dr. Goldenberg is a member of the American
College of Prosthodontists, the ADA and the Academy of Osseointegration.
He is also a Fellow in the ICOI and a Master in the IPS section of the ICOI. Dr. Goldenberg does some consulting for Dentsply Friadent.
Mark V. Larson, MD, is a Consultant in the Division of Gastroenterology and Director of Gastrointestinal Endoscopy at the Mayo Clinic in Rochester, Minnesota. He is an Assistant Professor of Medicine and also lectures extensively nationally and internationally. Dr. Larson is active in the American Gastroenterologic Association and his areas of special interest are gastrointestinal bleeding and gastrointestinal malignancies. He and his wife, Dr. Amy Williams, have been with the Mayo Clinic for nearly two decades.

John D. McDowell, DDS, MS, presently serves as the Director of Oral Medicine and Forensic Sciences at the University of Colorado School of Dental Medicine. He also holds academic appointments in the departments of medicine at the University of Colorado School of Medicine and the University of Utah School of Medicine. Dr. McDowell is also a fellow of the American College of Dentists and the International College of Dentists. Dr. McDowell has received multiple awards recognizing his teaching skills.
Brad J. Potter, DDS, MS, is Associate Dean for Academic Affairs at the University of Colorado Denver School of Dental Medicine. He teaches the Oral and Maxillofacial Radiology curriculum in the Department of Diagnostic and Biologic Sciences. Dr. Potter is a diplomate of the American Board of Oral and Maxillofacial Radiology, fellow of the American College of Dentists, and member of the International College of Dentists and the American Academy of Oral and Maxillofacial Radiology.
Stephen R. Snow, DDS, is a Visiting Professor at the UCLA Center for Esthetic Dentistry and is on the faculty of Esthetic Professionals in Woodland Hills, CA. The founder of P.E.R.F.E.C.T. Perspectives: Advanced Dental Education, a focused dental seminar series, Dr. Snow has lectured nationally on smile design, cosmetic restorative techniques, dental photography, and cosmetic imaging as well as maintained a full-time private practice in Danville, CA. He serves without remuneration as consultant to numerous manufacturers in the development of dental computer software, intraoral photography and smile imaging.
John Vaselaney, DDS, ARM, is Assistant Vice President of CNA HealthPro
and the national director of CNA's dental risk management program. He
has both written and lectured extensively on various risk management
issues that face today's dentists. In addition to his clinical
activities, he has served as attending faculty of a GPR program, a
director of quality assurance for a hospital dental department, and as a
consultant to the insurance industry. He holds the insurance designation
of Associate in Risk Management. Dentists attending this course and
insured by CNA through the Professional Protector Plan for Dentists
will be eligible for a premium credit.
Amy W. Williams, MD, is a Consultant in the Division of Nephrology, and Vice-Chair of the Department of Internal Medicine at the Mayo Clinic in Rochestor, Minnesota. She is an Associate Professor of Medicine and also lectures extensively nationally and internationally. Dr. Williams is on the American Society of Nephrology’s Women in Nephrology Committee and her areas of special interest are acute and chronic renal failure, dialysis, and metabolic and acid-base disorders.

Credit Hours
This course has been registered for 18 units of continuing
education credits for each week. We are approved as a continuing education
sponsor by the examining boards in California and other
states. Dental Seminars & Symposia is designated as an Approved PACE Program
Provider by the Academy of General Dentistry. The formal continuing education
programs of this program provider are accepted by AGD for Fellowship, Mastership
and membership maintenance credit. Approval does not imply acceptance by a
state or provincial board of dentistry or AGD endorsement. The current term of approval extends
from 5/2/1988 to 5/31/10.
Course Objectives Are:
- Teach current techniques and technology applicable to general dentists that have been substantiated by scientific research.
- Answer questions about procedures, products, and technology dentists are currently using and develop an awareness of alternatives.
- Establish and utilize university dental centers as resources for knowledge.
- Promote camaraderie among participants sharing ideas and contributing to a respected profession which
continues to contribute to the overall quality of life of our communities.
- Encourage excellence in dentistry.
- Help dental professionals meet their continuing dental education requirements.
Tuition & Deposit
Registration fee for each attending doctor is $880.00 for
the first week and an additional $465.00 for the second week (see
Special Savings For Hotel Guests). Any spouse or
staff member who wishes to attend the seminar with their accompanying dentist
will be charged $395.00 per person for the first week and an additional $235.00
for the second week. The first staff member from an office attending without
an accompanying dentist must pay the doctor fee. Multiple dentists from the
same office must each pay the doctor fee. Registration fee includes notebooks,
morning refreshment breaks, certificates of attendance and other materials
that may be provided. A $25.00 non-refundable deposit should be sent with your registration for
each individual attending in order to reserve a space at the forum. All fees
quoted are in U.S. dollars.

Refund & Final Payment
The $25.00 deposit for each individual attending the seminar
is not refundable. There are no additional seminar cancellation penalties.
Final payment should be made by January 3, 2010. Make your tuition check
payable to Dental Seminars and Symposia.
Charging Tuition Payment
You may charge your tuition to your Visa, MasterCard, or American Express. No additional charge is made
for this service. The refund policy is the same, except your refund will be credited to
your charge card account.
Tax Deductibility
This program is structured for tax-deductibility. Please check with your tax advisor.
Travel Information
Special rates have been obtained for you on airfares, hotel accommodations
and car rentals. Exceptional rates are available at the participating
Hyatts, if booked through Travel Leaders (formally Chippewa Valley
Travel). United Airlines is the official carrier for the seminar.
Airline costs vary depending upon your city of departure: call Travel
Leaders for the best airfare from your home city. We recommend purchasing
your airline tickets well in advance in order to secure the best
price.
The
cost for hotel accommodations begins at $1,285 per person, based
on double occupancy for a seven night package.
This includes hotel accommodations and related taxes. Luxury condominiums
are available for those who do not want to stay at the full-service
hotels. A minimum $500.00 per room deposit is required with your
reservation; the balance is due 50 days prior to departure. An administrative
fee of $100.00 will be assessed for any cancellation. Cancellations
received 90 days or less prior to departure are subject to any additional
penalties imposed by the suppliers. Trip insurance is recommended.
